Step 01 - Navigate to the Employee Voice Module and go to the Templates
Step 02 - Click on the "Add Template" button. Once clicked, the following screen will appear. Name your Template and then click "Save".
Step 03 - Upon clicking, the following screen will appear.
In this form, there are three main sections:
Widgets – Contains the standard form elements you can drag and drop.
Form Design – The area where you build and organize the layout of your form.
Field Properties – Displays the configuration options for each selected field within the form design.
These are the pre-defined fields available in the Template. They are system-generated and serve as the foundational elements required for capturing standard case information.
You can drag and drop widgets into the Form and customize the layout and fields to suit your organization's requirements.
Step 04 - Use the table below as a reference when completing this form.
| Field Name | Description |
| Title | A brief, clear title summarizing the nature of the feedback. |
| Description | A detailed explanation of the grievance, including relevant context and background information. |
| Date of Incident | The specific date when the incident or issue occurred. |
| Attachment | Upload any supporting documents or evidence related to the grievance, such as screenshots, emails, or reports. |
| Priority | Indicate the urgency or importance of the grievance (e.g., Low, Medium, High). |
| HR/Action Owner Comments | Section for HR personnel or assigned action owners to add remarks, actions taken, or follow-up notes related to the grievance resolution process. |
Step 05 - For every field you add to the Template design, you can configure specific properties.
For example, for the Employee Autocomplete widget, you can configure properties such as adding placeholders, adding multiple employees, and displaying employee profile pictures upon auto-completing.
Step 06 - Also, you can specify whether the validations are required or not for a certain field. After making these changes, click the "Save" button.
Step 07 - Click the "Preview" button to preview your template design before publishing.
